David – age 39 Loves music especially classic rock Weekly visit with his brother Ron, and chats with him at least twice a week by phone Favourite foods are, potatoes, chicken, salad, ice cream, and chips Enjoys listening to her radio, taking walks, and musical performances Lives in a group home with 5 other individuals since the age of 17 Completes activities of daily living with moderate support, only requires prompts for hygiene activities Attends a recreational day program that supports building life skills, leisure activities, and recreational activitiesUnique Considerations:• Developmental Disability – cognitive level age 7- 10• Tubular Sclerosis• Brain tumour that was diagnosed in 1989, after David had a seizure. Purpose and Audience Assignment # 1Due Tuesday September 17thReview the below noted scenario of David’s evening at his group home, and compose a note to your co-workers, and then write his evening shift report/journal entry/log.David’s EveningDavid arrives home from his day program at 4:15pm. He comes into the house and goes straight to his room to put his belonging away. He then comes back out to the dining room and sits in his favourite chair with his transistor radio. He puts on his headphones to listen to music. When staff asks David about his day, he turns his head and avoids eye contact. However, when staff asks him if he would like a cup of tea and a small snack he looks directly at the staff member and nods yes. The staff member asks David if he can use his words to choose a snack. It takes David a few minutes to respond with “crackers please”. He eats his snack and drinks the coffee while listening to music.At dinner time David joins his peers at the table without being prompted for a dinner of chicken pot pie, a dinner roll, water and tea. It is David’s habit to eat one ingredient of a meal at a time; he does this while eating the chicken pot pie, pulling out each ingredient until it is gone then moving onto the next one. He finishes his meal then sits quietly at the table until prompted to bring his dishes into the kitchen and to put them into the dishwasher.After putting his dishes in the kitchen David goes back to his favourite chair to listen to his music. A staff member gently reminds him that he needs to complete his chores for the evening the he can relax with his radio. David responds by once again by turning his head and avoiding eye contact with the staff member. The staff member gives him a few minutes then returns with his task pictures and asks David which tasks he would like to complete first, and hold out three task pictures (shower, laundry, & make work lunch) for him to look at. David studies the pictures for a few minutes then points to the shower picture. He immediately takes off his headphones and heads to his room to gather his shower supplies. He completes his shower, redresses, then gathers up his laundry and brings it to the dining room. When he sees that no one is around he sets the basket down, then goes to back to his room retrieves his radio then settles into his chair with his headphones on.
When a staff member comes into the dining room and sees David listening to his radio and his basket of laundry sitting by the table the staff member praises David for bringing out his laundry and asks him if he wants to put the laundry on to wash or make his work lunch.David responds with “make my lunch”. Before they even head to the kitchen a commotion starts in another area of the house which requires the staff to assist his co-worker. David settles back down into his chair to listen to his radio until the staff comes back to assist.When the staff returns he sees the basket, picks it up and says to David“let’s get this going, okay?” Davis nods and goes with the staff to put on the laundry.After the laundry is put on to wash the staff member asks David if he wants to go toTim Horton’s with two of his house-mates.
